Authorization to Release Medical Records

At Tower Imaging Valencia, we believe you should have easy access to your imaging reports and scans. Whether you need to view your digital reports via the Henry Mayo MyHealth Portal or prefer physical media, our medical records process is secure, simple, and designed with your convenience in mind.

How to Access Your Records

  • Online Portal: Patients with a Henry Mayo Newhall Hospital MyHealth account can view interpreted results for all studies performed at Tower Imaging Valencia.
  • CD or Printed Reports: Visit us in person with a photo ID during business hours to pick up physical copies of your records.
  • Requests from Other Facilities: Fill out an Authorization to Release Medical Records form and email it to tivregistration@henrymayo.com to have your records transferred.
  • Kaiser Patients: Finalized studies are sent electronically to Kaiser. Contact Kaiser Medical Records at (818) 375-2208 to request your copies.
